APPLICATION ENCLOSURES

The application may be submitted in the form (http://www.nconzo.cz/web/registr/16), in Czech language.
The applicant is required to submit evidence of professional education including qualification documents and details of basic and post-basic education. The application should contain the following:
Officially authenticated translation into Czech of evidence of physiotherapy qualification (diploma)
Documentation regarding further education (copies of certificates of CPD activities)
Evidence of exercise of a profession
From education institution – details regarding the education program (subjects and number of hours in each subject)
Certificate of vocation eligibility (health) – not older than 3 months
Evidence of conviction record (not older than 3 months)

LANGUAGE APTITUDE TEST RECOGNITION
|
|
The migrant must achieve knowledge in the Czech language.
Aptitude test and/or adaptation period will be applied individually if there is a significant difference in the physiotherapy education of the applicant and the Czech republic education system.
